In modern, busy organizations of today, a defect or a problem may be observed so often that it fails to generate a question. Because it is so “normal’ to expect the problem, questions go unasked.
Take for example attendance to an important announcement meeting. It is normal to expect not everyone to be available. It is normal not to expect everyone to come on time! It is normal to expect the program to start 10 mins late if the chief guest/chair person is the CEO, etc…
Listing all “normal” problems in an organization, establishes normalization of the defect!
Solution is to ask questions! – the first time and every time the problem is noticed or detected! Even if there are no answers at times, things will change for good!
All normalized defects should be questioned, routinely! Building this culture in our teams can enhance the performance of the organization greatly – with not much of an effort and fanfare of initiatives!
